*Permanent Social Services Director - Columbia, SC*

New management seeks experienced Social Services Director (SSD, DSS) for a Skilled Nursing Facility (SNF) in the greater Columbia, South Carolina area. Ideally we aim to bring this provider a capable and experienced individual who has strong awareness and knowledge of available social services that support seniors and veterans in SC as well as an ability to oversee and support a team.

To be eligible for this job, candidates must be licensed as a Social Service Worker (SSW) and/ or Certified Social Worker (CSW) and/ or Licensed Clinical Social Worker (LCSW) in SC .

What the job entails :

• For each new resident, you'll meet to develop a social history, social assessment and care plan which identifies issues and needs, so that realistic goals can be defined and specific action can be taken to improve things.
• You'll be an integral part of interdisciplinary Care Plan meetings.
• You will help the resident and resident’s family in discharge and placement planning.
• Coordinate family groups to promote open communication, education and support between family members, facility staff and administration, and provide counseling as needed.
• You'll educate the community regarding aging, rights of residents, facility services and more.
• You'll help screen all potential residents prior to admission and provide information to residents and families during the admission process, including providing orientation that helps the resident gain comfort in the long-term care environment.

What you need to bring to the job :

• Positive attitude about working with long-term care residents and elderly
• License as a Social Service Worker (SSW) and/ or Certified Social Worker (CSW) and/ or Licensed Clinical Social Worker (LCSW) in South Carolina
• Previous Social Services background in a Skilled Nursing Facility setting and awareness of State and Federal regulations for SNFs
• Computer savvy is essential in order to keep up with documentation demands
• Communication skills are essential to express things clearly to a variety of individuals from all walks of life
• An ability to educate and mentor others as a leader is vital in this role
